One prominent change to the classic items is that the Fake Item Box is now able to block shells again, after being unable to do this in Dry Bones Kart: Double Dash!! to Dry Bones Kart Wii.

Fake Item Box - Mario Kart 7 Fake Item Box This item imitates an Item Box, but is colored red and has an upside down question mark. It causes opponents to flip over once hit. It can block item attacks.
POWBlockMK7 POW Block Causes all racers ahead of the user to spin out and drop their items if used. They can avoid this item by either being in the air or pressing the use items button.
LightningCloudMK7 Lightning Cloud A thundercloud hovers over the recipient and strikes it with lightning after ten seconds. The user shrinks to very small size for 10 seconds, driving more slowly. Before this happens, the user has higher speed and acceleration while not losing speed when driving off-road, and may pass the cloud to an opponent by bumping into them.
Banana Peel - Mario Kart Wii Banana Peel Players that run over Bananas spin out.
Triple Bananas - Mario Kart Wii Triple Banana Gives the player three Bananas that trail behind the kart. They can then be dropped or thrown one by one.
Banana Bunch - Mario Kart Wii Banana Bunch This item gives the driver a trail of five bananas. Causes the five bananas to go flying, plays a crazy sound effect, and flips over the kart.
Green Shell - Mario Kart Wii Green Shell Travels in a straight line and bounces off walls. Knocks over the player it hits.
Triple Green Shells - Mario Kart Wii Triple Green Shell Three Green Shells that protect the player from incoming attacks by rotating around its kart. They can then be thrown one by one.
Red Shell - Mario Kart Wii Red Shell Chases after the closest player in front of the player and knocks it over. Throwing it backwards will make it travel in a straight line, like a Green Shell.
Triple Red Shells - Mario Kart Wii Triple Red Shell Three red shells that protect the player from incoming attacks by rotating around its kart. They can then be thrown one by one. CPU players can't get this item in Grand Prix and VS modes.
WingedSpinyBombshellMK7 Spiny Bombshell

Flies over the other racers all the way to the player in the lead, then explodes on him, blasting him into the air.
MegaMushroomMK7 Mega Mushroom
Unused Item
The driver grows bigger, which allows them to squash other racers by running over them, making them lose their items and slow down for a moment. The Mega Mushroom makes the racer invincible to all attacks except those from Stars, Bullet Bills, and Bloopers. It allows the user destroy items and course obstacles and gives the driver a speed boost. In addition, the player can go off-road without suffering any effect of slowing down. The effect lasts for 7.5 seconds but will be ended prematurely if the player is hit by a Lightning Bolt.

A blue shell that flies directly to the racer in first place and explodes like a Bob-omb.
Bob-omb - Mario Kart Wii Bob-omb After being thrown or dropped, it explodes after a moment or when a player comes too close, blasting any kart into the air that gets caught in its blast radius. It will also chase after players that comes by.
Mushroom - Mario Kart Wii Mushroom Gives the player a speed boost.
Triple Mushrooms - Mario Kart Wii Triple Mushroom Same effect as the Mushroom. It can be used three times.
Golden Mushroom - Mario Kart Wii Golden Mushroom Same effect as the Mushroom. It can be used at will, but only for a moment after the first use.
Bullet Bill - Mario Kart 7 Bullet Bill Transforms the player into a Bullet Bill, which rockets down the entire track at high speeds on auto-pilot for a time that is determined by the player's current position. The Bullet Bill is invincible and can knock over items, course hazards and other players, which makes them lose any item in their possession.
BlooperMK7 Blooper Sprays ink on all the players ahead of the user and reduces their visibility. The ink will go away after a moment or when the player receives a boost from a Mushroom or a Boost Pad.
Lightning Bolt - Mario Kart 7 Lightning Bolt Causes all players, except the user, to drop their item, shrink, and have their kart's top speed reduced for a moment, the effect being longer for the players that are leading the race. They can also get squashed by normal-sized players.
Starman - Mario Kart Wii Starman The player obtains total invincibility for a moment and is able to knock over everything in its path including items, course obstacles and other players, making them lose their current item. In addition, the player receives a speed boost and the ability to drive off-road without losing any speed.
Fire Flower - Mario Kart Wii Fire Flower
New Item
Allows the player to throw fireballs that bounces off walls and cause other players to spin out of control on impact. Up to three fireballs are allowed at a time per player, throwing a fourth one will make the oldest one disappear. If ten fireballs are used, this item wears off immediately.
Super Leaf - Mario Kart Wii Super Leaf
New Item
Gives the player's kart a tail that can be used to deflect items, knock over opponents and collect coins. The tail will disappear after a moment or if the player is hurt.
Lucky Seven - Mario Kart 7 Lucky Seven
New Item
Surrounds the player with seven items that rotates around its kart. The player receives a Mushroom, Banana, Green Shell, Red Shell, Blooper, Bob-omb, and a Star. CPU players can't get this item.
